Introduction: Graft-versus-host disease (GVHD) presents high mortality and morbidity, occurring in 30-60% of patients receiving allogeneic bone marrow transplant¹. Among those who develop acute GVHD, 50% will perish². Recently, studies have suggested that GVHD patients also present heart damage, even in acute GVHD ³,4. Baring this in mind, the cannabinoid system is known to modulate inflammation, proliferation, differentiation, cellular survival5,6, and cardioprotective processes7. Therefore, it is interesting to evaluate if GVHD causes inflammation and damage to the heart, and if interventions with cannabinoids can protect the heart. METHODS: GVHD was induced in Balb/c mice by the transplant of 1x107 bone marrow cells and 3x107 splenocytes from C57BL/6j mice, while syngeneic mice received cells from Balb-c donor. Mice were treated with Cannabidiol (CBD; 30mg/kg), or vehicle daily. Mortality and clinical score were assessed. At the 7th day mice were euthanized, and the heart was collected for morphometrical analysis, quantification of cytokines/chemokines (ELISA), and bacterial translocation assay were performed. RESULTS: GVHD mice exhibited high mortality when compared with syngeneic group (0,0018) and treatment with CBD increased survival (0,0066). GVHD mice displayed increased cardiac CCL2 (0,0270) and CCL3 (0,0126) when compared to syngeneic mice, while CBD treatment prevented CCL3 increase (0,0278). GVHD mice have higher presence of translocating bacteria in blood (0,0489). Finally, we found no morphometrical alterations in the heart, although the ratio of heart and mice weight was reduced in GVHD mice (0,0170) and CBD prevented it (0,0012). CONCLUSION: Although these are preliminary results, GVHD impacted mortality of mice, and this was prevented by treatment with CBD. GVHD mice displayed higher levels of CCL2 and CCL3, and CBD was able to prevent CCL3 increase also maintaining heart/body mass ratio. Furthermore, more data is required to fully comprehend the implications of GVHD in the cardiovascular system and if CBD can prevent it.
