Introduction: Cancer represents one of the leading causes of morbidity and mortality in the country, with projections indicating a sustained increase in incidence, particularly in less developed regions. The North region, especially the state of Pará, presents unique challenges related to access, early diagnosis, and continuity of care, which may contribute to underreporting and regional disparities. Objectives: To analyze the evolution of new cancer cases in the state of Pará between 2020 and 2024, comparing them with national data to identify highlighting regional disparities. Methods: This is a descriptive, retrospective, and quantitative study based on the analysis of secondary data from the Oncology Treatment Monitoring Panel (Painel-Oncologia/DATASUS), covering the period from 2020 to 2024. The study includes data from the Outpatient Information System (SIA/SUS), Hospital Information System (SIH/SUS), and Cancer Information System (SISCAN), with information categorized by sex, age group, and type of diagnosis according to the state of residence. Descriptive statistics were applied to calculate absolute and relative frequencies and percentage variations, with a comparative focus between the state of Pará and national trends. Results: Between 2020 and 2024, cancer cases in Pará showed a consistent increase, rising from 7,119 to 10,266 cases—an increase of 44.2%. This rise was more significant among women, with a 52.4% increase (from 4,191 to 6,387), especially between 2022 and 2023 (+11.8%). Among men, the increase was 32.5% (from 2,928 to 3,879), with the most substantial rise occurring between 2020 and 2021 (+15.7%). Most cases were concentrated in individuals over 55 years of age, a pattern also observed at the national level. In comparison, Brazil recorded a more modest increase of 14.9% over the same period (from 509,196 to 584,998 cases), peaking in 2023 (673,989) before declining in 2024, particularly among men (-15.8%) and women (-11.3%). Regarding types of diagnoses, malignant neoplasms (as defined by Law No. 12.732/12) increased by 42.8% in Pará (from 4,974 to 7,103), while nationally, there was a 4.4% decrease (from 356,659 to 340,906). In situ neoplasms showed the highest proportional increase in Pará, rising 123.5% (from 379 to 847), particularly between 2022 and 2023 (+67.8%), compared to a 68.0% increase nationwide. Neoplasms of uncertain or unknown behavior increased by 26.3% in Pará (from 1,047 to 1,322), while at the national level the increase was 73.8%. Cases under codes C44 and C73 (non-melanoma skin cancer and thyroid) doubled in Pará between 2021 and 2024 (from 497 to 994), in line with the national growth trend (37.6%). Conclusion: The findings reveal a significant and continuous increase in cancer cases in Pará between 2020 and 2024, especially among women and early-stage diagnoses. In contrast to the national stabilization trend, the results suggest local improvements in detection and possible shifts in the regional epidemiological profile.
